This dashboard profiles the present and previous end of quarters position against a range of measures, some coverage, some clinical in nature. Included with the dashboard are drill down facilities to generate lists of patients who are "in category" and separately "not in category". The dashboard therefore supports not only a vehicle for retrospective assessment of the quality of service, it also supports proactive performance support. So for example, Patients who require an intervention can be readily identified e.g. those patients with Diabetes who have not had a Diabetic Annual Review or in a certain age, sex group and without a CHD primary risk assessment undertaken. The LDP Dashboard report also benchmarks Practices, other Practices in their Locality, against any defined Locality, PBC Group/Consortium and PCT.

The LDP Reporting Dashboard is currently available and deployed. It is available within the Demonstration Site.

This dashboard profiles the data extracted from General Practice Clinical systems against data quality standards of coverage, completeness, up to date(ness) and internal consistency. The scope includes standard data quality assessment against administrative and clinical data and against the GMS contract's IM&T DES. The Data Quality Report supports diagnosis by highlighting inconsistencies in the data, for example, a person with three consecutive BP readings over 140/90 who does not have a diagnosis of Hypertension.

The Data Quality Dashboard is specified and is planned for delivery in August 2007 (this may be delivered earlier if ordered. Its development is speculative at present).

This dashboard is related to the Data Quality Dashboard, however its focus is on the identification of patients whose data suggests a diagnosis should be made. It also identifies patients who are at risk of a range of conditions. In terms of improving the outcomes for patients, establishing a diagnosis is one of the most significant interventions that can be made. Several diagnoses are determined definitively by laboratory test results; the present of existing prescribing data; certain examinations; and the recording of unusual/non-standard "read codes". All result in this dashboard prompt clinicians to intervene and formally establish a diagnosis and associated care plan.

The Improving Diagnosis Dashboard is specified, focuses on Long Term Conditions and is planned for delivery in August 2007 (this may be delivered earlier if ordered. Its development is speculative at present).

This dashboard acknowledges that care is provided by multiple agencies and no one provider is fully informed about their patients' current status, risk, administrative and clinical. The structure of the CDRIntell web-based system support an understanding about what a given organisation does not know about their patient. CDRIntell can therefore be configured to proactively and electronically inform organisations of key updates about their patients. For example, General Practice may be informed of inpatient stays by their patients and associated diagnosis and procedures undertaken; key laboratory test results; and examinations undertaken by community services. Only where the Pracitce has not recorded this data will the notifications be generated.

In order for this service to be of value, hospital and community services data will need to be loaded along side primary care data into the CDRIntell repository.

The Notification Service Dashboard allows the definition of the updates that are required and provides a profile of the updates not yet applied. So for example, for key data items it will be possible to profile the patients based on Primary Care Sourced Data, Hospital Sourced Data and Community Sourced Data. Only data not recorded by your organisation will be presented and there will be a choice of all data or just data within the last 15 months.

The benefits of this approach are self evident, however just to illustrate. In a study conducted by Health Intelligence in West London examining way to reduce risk - we examined cardiovascular related In-patient stays and discovered re-admission rates were 50% less where the Practice knew of the In-patient event. Where examinations are undertaken by other organisations, and made available to your organisation, there is no need to undertake the procedure again. This therefore changes the focus to ensure those that have not been addressed by anyone can be focused on.

The Notification Service Dashboard is in the process of being specified. Health Intelligence is seeking a customer to pilot this valuable service. It will be of interest to organisations wishing to "invest to save".

This dashboard focuses on Childhood Vaccination and Immunisations. It details by Practice, defined Localities, PBC Groups/Cluster and the PCT(s) the current position and the end of each quarter positions. It supports not only an analysis of performance; it also details children due in the following quarter to support their invitation to participate in the programme.

The Vaccination and Immunisation Dashboard is currently being piloted and will be available during June 2007. This beta version is available within the Demonstration Site.

This dashboard focuses on referrals made by General Practice to the Hospital Sector. At present referrals may use Choose & Book, they may also be made directly or via a Referral Management Centre or similar. Given the eighteen week wait initiative it is more important than even to establish an early position on the referral demand. It is important to understand this at a Practice, PBC Group/Cluster Level and a PCT level. It is also of interest to determine of those referred to each speciality which resulted in an episode of care and to understand the associated outcomes. Where the Secondary Uses Service (SUS) commissioning data is loaded into CDRIntell, the hospital episode may be tied back to the original referral and analysed.

The Specification work for this Dashboard is underway and is due to be completed during June 2007 for deployment in August.

This dashboard takes each NSF Standard for Diabetes and presents a comprehensive assessment for the Practice, defined Locality, PBC Groups/Clusters and the PCT. Patients who have not had examinations, tests, or who are not prescribed as recommended are readily identified. Patients with the secondary complications associated with diabetes are highlighted and those at risk. A Blue, Red, Amber and Green (BRAG) dashboard is presented for each Standard with drill down functionality to the individual record (for authorised users).

This Diabetes NSF Dashboard has been in use for three years, however has recently has a face lift. Available within the Demonstration Site.

This dashboard takes each NSF Standard for Coronary Heart Disease and presents a comprehensive assessment for the Practice, defined Localities, PBC Groups/Clusters and the PCT. The approach taken in the Diabetes NSF is mirrored for this Dashboard.

The CHD NSF Dashboard has been in use for three years, however is undergoing a re-vamp of the menu structure to match the approach for the Diabetes NSF. This is due to be completed in July 2007.
