Approximately 20 million people in England, almost one in three of the population, suffer from a Long Term Condition.

Patients with Long Term Conditions are very intensive users of health care services using 50% of all GP appointments and 70% of all inpatient bed days.

The UK economy stands to lose £16 billion over the next 10 years through premature deaths due to heart disease, stroke and diabetes.

Current policy on Long Term Conditions (LTC) seeks to empower patients by giving them information and guidance about their condition. The generic LTC model places the emphasis on engaging the individual in making shared decisions about their care.
